Microsoft Word Template - Kinematic Concepts

In order for a teacher to validate student-earned Success Codes, it is important that they be sent to the teacher with the proper formatting. The Microsoft Word file below has the proper formatting.  Students must enter their Name, the ID number that have been using when doing Minds On Physics, and the Teacher Code that they have been using when doing Minds on Physics. Finally, students must enter all success codes on the proper line. There must be at least one space between the colon (like the one after KC1) and the success code. Success codes must be entered with all eight letters and the hyphens in the appropriate locations. This includes the initial character - either a "g", "s", or a "b". Finally, it is important that there be no spaces mixed in with the eight letters.
